How to Apply
If you want the Board to review the children’s aid society’s or licensed adoption agency’s decision, please print and complete the appropriate application form.

Your application should be sent to the office of the Child and Family Services Review Board within 10 days of you receiving the children’s aid society or licensed adoption agency’s letter informing you of its decision. Your application should include a copy of the children’s aid society’s or licensed adoption agency’s letter advising you of its decision.
In exceptional circumstances, the Board, at your request, may extend the 10 day period for submitting applications.
