How to Apply
If you want the Board to review the children’s aid society’s decision, please print and complete the application form.

Your application should be sent to the office of the Child and Family Services Review Board within 10 days of you receiving the children’s aid society’s letter informing you of its decision. Your application should include a copy of the children’s aid society’s letter advising you of its intention to remove the foster child.
In exceptional circumstances, the Board, at your request, may extend the 10 day period for submitting applications.
