How to Apply

If you want the Board to review a children’s aid society’s decision, please print and complete the application form.

When completing your application, please be as precise as possible when providing information about your complaint or concern. In order to assist the Board in understanding your concerns, please include information about the facts of your particular situation, dates of incidents that you are referring to, and copies of any correspondence that you have received from the society to support your request for a review by the Child and Family Services Board.
