How to Apply

If you are interested in having your application reviewed by the Custody Review Board, please call (416) 327-4673 or 1-888-728-8823 or (TTY) (416) 327-9247 and ask to speak with a Case Coordinator.

The Case Coordinator will complete a telephone application with you.

The Case Coordinator will ask for the following information:

1. your name;
2. your date of birth;
3. the name and telephone number of your current facility;
4. the name of the unit within your current facility;
5. the name of your primary worker at the facility;
6. the name of your Advocate at the Office of the Provincial Child Advocate, if applicable;
7. the name and phone number of your Probation Officer, if applicable;
8. the date you were placed or transferred to your current facility or the date you received a decision from a Provincial Director regarding your placement, transfer, temporary release or reintegration leave;
9. the reasons for your request to have your application reviewed by the Custody Review Board; and
10. specific information about previous and outstanding charges and your current sentence.

You must contact the Custody Review Board within 30 days of your placement in or transfer to a custody facility or within 30 days of receiving a Provincial Director’s decision about your placement, transfer, temporary release or reintegration leave.
